Please complete all sections of this form. If you do not know your student number this can be added at a later time.


In order to assess this application proof of income must be provided, please see below examples of evidence you can provide:

  • Universal Credits (most recent payment statement showing full breakdown)
  • Personal Independent Payments Award Letter (must be in receipt of another benefit alongside)
  • P60 2019-2020
  • Income Support**
  • Last 3 wage slips (paid monthly)
  • Last 4 wage slips (paid weekly / fortnightly)
  • Job Seekers Allowance**
  • Employment Support**
  • Pension Guarantee**
  • Disability Living Allowance** (must be in receipt of another benefit alongside)
  • Local Authority letter if you are in or have Left Care
  • Working / Child Tax Credit Award Notice (headed Tax Credit Award April 2021-April 2022 and will show income for 2020-2021 on page 2 or 3)

**this can be either a letter dated within 3 months stating you are in receipt of the benefit or alternatively you can provide a bank statement dated within 3 months showing your name and address along with 3 or more payments being paid into your account.


As well as proof of income you also need to provide electronic copies of the following evidence before your application can be assessed:

  • Full birth certificate for all children that you are apply for funding
  • Ofsted Registration Certificate (childcare provider should send this – please see below for further information)

    If you are a student aged 20+ please provide the following information. Please be advised that childcare will only be paid for the days that you are timetabled at College or on mandatory placement (confirmation of placement days will need to be provided by your tutor). Students who are aged 19 years old need to apply for Childcare funding through Care to Learn via the website.

  • Your childcare provider will need to complete a form which you can ask Student Services to post to you by calling 01946 839300 or emailing If you use more than one provider, you must ask for a copy of the form and send all Childcare Provider information forms back to student finance at the College. All childcare providers MUST be OFSTED REGISTERED.

  • If you do not have an email address you can hand a copy of your evidence into Student Services at the College (please do not post the application and evidence to us). You can submit the application at any time but payments will only commence once you have enrolled, we have checked and approved your eligibility and you are attending classes.
  • Following receipt of your application, you will be informed of the outcome of your application by email or text in due course.
